Description

The Outcomes Manager reflects the mission, vision, and values of NM, adheres to the organization's Code of Ethics and Corporate Compliance Program, and complies with all relevant policies, procedures, guidelines and all other regulatory and accreditation standards.

The principal role of the Outcomes Manager in Patient Care Services Nursing Professional Practice & Development is to assist the patient care team in achieving the highest quality and best possible outcome for the patient in a cost effective manner. The Outcomes Manager collaborates with members of the health care team across the continuum. The Outcomes Manager demonstrates a thorough understanding of the demographics for the hospital. Develops approaches to optimize the equity, effectiveness and efficiency of the delivery of resources available to the population both within and beyond the acute care setting.

Description

  • Coordinates all activities related to NM Palos Hospital (PH) nursing professional practice outcomes data analysis and management.
  • Represents NM PH on quality committees.
  • Coordinates CQI activities at NM PH in conjunction with performance improvement committees.
  • Represents NM PH at administrative meetings and brings all information back and shares with the division.
  • Collaborates with professional practice development consultants and interdisciplinary teams to ensure that there is a link between mandated education and quality initiatives.
  • Coordinates state and regulatory programs and facilitates quality improvement initiatives with these programs.
  • Coordinates and facilitates regulatory and accreditation visit outcomes and related documentation.
  • Uses internal and external benchmark data and best practices to establish specific targets, timelines and metrics for improvement projects; provides communication and update on progress
  • Encourages those who identify problems, and participate in suggesting solutions to these problems Supports staff in piloting new ideas and solutions, within or across departments. Recognizes, rewards, and celebrates improvements and staff's contributions
  • Creates an environment of partnership, collaboration, seamless quality and service enhancement across team, departments and divisions of the organization
  • Designs and implements database queries and reports
  • Manages quality and outcomes data (including databases) to respond to customer inquiry
  • Provides appropriate analysis of data as evidenced by customer satisfaction
  • Conducts studies/collects data related to the delivery of patient care
  • Utilizes applicable resources and information systems to obtain data and reprt outcomes
  • Coordinates committees, acts as a resource to staff for research and EBP projects.
  • Serves on hospital/nursing councils and serves as a resource for unit based quality councils.

Qualifications

Required:

  • Bachelors of Science in Nursing
  • RN License
  • Two to five years' experience with the study of patient populations and working with care providers
  • CPR - Basic Life Support
  • Proficient with MS Office Products, primarily Excel and PowerPoint

Preferred:

  • Previous leadership experience
  • Preferred CPHQ or other quality certification
  • Masters in nursing or related field
