Job Description:

The Community/Hospice Chaplain works within the homecare/hospice setting providing emotional, spiritual, and religious care for patients, families, and staff. The Chaplain is responsible to utilize effective communication skills with all patients, volunteers, and staff in a timely, concise, clear, and positive manner.

This position is based in Weber county.

Monday - Friday 8 AM - 5 PM. No Weekends or Holidays.

Scope of the Chaplain role:

This position adheres to the Joint Commission standards of Spiritual Care and the Common Standards of Practice for Professional Chaplains. Patient and caregiver experience is improved through a variety of means including active listening, supportive dialogue, prayer, and meditation. Chaplains provide support during a wide variety of clinical scenarios including chronic illness, sudden changes in health status, adjustments to new and chronic illness, and end-of-life situations (including codes whenever possible). They coordinate with local clergy for religious rituals and sacraments as needed and may assist with advance directive preparation and advance care planning. Chaplains may lead or coordinate Intermountain caregiver self-care activities, including individual work with caregivers, staff outreach, and group support activities.

This position consults and collaborates with other members of the interdisciplinary team and contributes to the patient plan of care as appropriate, providing a spiritual perspective on ethical decision-making related to treatment and other health care issues as appropriate. Chaplains demonstrate continued professional growth and development through participation in education programs and review of current literature and develop, lead, and/or participate in appropriate support groups as appropriate. These duties are performed in a manner that assures that all documentation meets requirements for accuracy and timeliness, and reflects services provided, billing requirements, patient/caregiver needs, and confidentiality. Chaplains maintain confidentiality of business and health information in accordance with HIPAA and state regulations, and Intermountain policies.

Minimum Qualifications

One unit of Clinical Pastoral Education or currently enrolled in a CPE program.

Maintain ordination/commissioning and endorsement from individual's faith background. Verified upon hire and every five years.

One year of experience providing spiritual care in a health care setting.

Demonstrated familiarity with and respect for spiritual, religious and cultural diversity.

Demonstrated excellent organizational and interpersonal skills.

Demonstrated ability to communicate effectively orally, in writing, using technology.

Willing to work as part of a team in an interfaith, inter-spiritual, interdisciplinary multicultural setting.

For Homecare/Hospice Employees: Current driver's license, insured and reliable transportation and an acceptable driving record. Access to connect to the Internet for transferring data twice a day or able to commute to the office twice on scheduled days to transfer data.

Preferred Qualifications

Master of Divinity in a related field

Four or more units of Clinical Pastoral Education (CPE)

Three years of experience as a chaplain in a clinical setting.

Experience working with Palliative Care/Hospice patients.

Ability to understand, speak, write and read the Spanish language.

The hourly range for this position is listed below. Actual hourly rate dependent upon experience.

$23.94 - $37.69
