Salt Lake Behavioral Health is a private 118 bed freestanding psychiatric hospital specializing in mental health and substance abuse treatment for adolescents and adults. We serve the Salt Lake City and surrounding area using evidence-based treatments in a comfortable, confidential atmosphere.

We believe in an individualized, patient-focused approach to diagnosis and treatment. For some, anxiety or drug dependence looks different from others with the same diagnoses, and in some cases, Behavioral health and substance use disorders occur simultaneously. In these cases of dual-diagnosis, we treat the patients in the best way possible given their specific case.

Salt Lake Behavioral Health is currently seeking a compassionate and talented Outpatient Case Manager to join our team.

The Outpatient Case Manager is responsible for meeting with all eligible patients to discuss the benefits of Outpatient services as part of their continuum of care treatment. The Outpatient Case Manager will be responsible for communicating eligibility to key members of senior leadership & associated departments and will be responsible for routinely collecting and entering data, specific to Outpatient eligibility into Midas for trending report purposes. The Outpatient Case Manager will have a thorough understanding of the Step-Down Program. The Outpatient Case Manager, contacts external case managers/managed care organizations for certification and recertification of insurance benefits throughout the patient's stay, and assists the treatment team in understanding the insurance company's requirements for continued stay and discharge planning. The Outpatient Case Manager is responsible for having a thorough understanding of the patient's treatment through communication with the treatment team. The Outpatient Case Manager advocates for the patient's access to services during treatment team meetings and through individual physician contact. The Outpatient Case Manager chairs treatment team meetings and continued stay reviews as indicated.

Qualifications

Position Requirements:

  • Education: Bachelor's degree from an accredited college or university in social work, recreational
    therapy or mental health preferred.
  • Experience: Two (2) years' experience in a healthcare setting or managed care company, with
    experience in patient assessment, treatment planning, utilization management, and/or case
    management preferred but not required. A thorough understanding of the Inpatient workflow
    process, as it pertains to case management and discharge planning, is preferred.
  • Additional Requirements: CPR certification is required. You will need to successfully complete
    our current training for restraints and seclusions. This training will need to be done prior to
    assisting in any patient restraints or seclusions and within 90 days of employment.
